Who we are
SocialCare Partners LLC ("we", "us") coordinates medically tailored meals and grocery delivery for eligible New York Medicaid members under the NY State 1115 Medicaid Waiver Program. This policy explains what we collect through this website and how we use it.
Information we collect
- Contact details you give us: name, mobile phone number, email address, borough.
- Program details you choose to share: dietary needs, allergies, health or life circumstances relevant to eligibility, and any notes you add.
- Consent records: whether you opted in to text messages, the exact consent wording shown to you, and the date and time you agreed.
- Basic technical data such as the page you submitted from.
How we use your information
- To check your eligibility and enroll you in the meal program.
- To coordinate with your Social Care Network, Medicaid plan, or care manager.
- To build your menu and schedule and confirm weekly deliveries.
- To contact you by phone, email, or — if you opted in — text message.
- To meet legal, program, and recordkeeping obligations.
Text messaging and your phone number
If you check the SMS consent box, we may text you about your sign-up, eligibility, menu, and deliveries. Message frequency varies (roughly 2-6 messages per month). Message and data rates may apply. Reply STOP to opt out at any time or HELP for help. Opting in is never required to receive meals or any other service.
No mobile information or SMS consent will be sold, rented, or shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent are not shared with any third party except the messaging service providers that deliver our messages on our behalf.
When we share information
We share information only as needed to deliver the program: with your Social Care Network or Medicaid managed care plan, with delivery and meal preparation partners, with service providers who operate our systems (such as our messaging and hosting providers), and when required by law. We do not sell your personal information.
Data retention and security
We keep sign-up and consent records for as long as needed to run the program and meet legal requirements, and we protect them with reasonable administrative and technical safeguards.
Your choices
You can ask us to access, correct, or delete your information, or to stop contacting you, at any time. Text STOP to end messages, or contact us using the details below.
